Prospective patients should begin by filling out our contact form, located here. When Grace receives your contact information, she will reach out to you personally to schedule a consult. Currently, consults will be done in office or virtually if preferred. The goal is to simply get to know each other and to answer any questions you might have about the practice and begin enrollment.
